How do I join the CCRF?

If you are on active duty in the US Public Health Service Commissioned Corps you can apply by simply filling out the on-line application form. You must supply your PHS Serno and Social Security Number so they we can ensure that you are an officer. Additionally, you must join the CCRF Listserv.   This email listserver provides the primary mechanism that CCRF communicates with its members.

Once you have completed both of these steps, you will be a CCRF member in CANDIDATE status.   As you complete the different requirements of membership you will be able to advance to ROSTER-QUALIFIED and finally to FULLY-QUALIFIED when you will be eligible to receive the Field Medical Readiness Badge (FMRB).  For more information on CCRF membership requirements visit the CCRF Basics section of this website.
