- The
following are the only acceptable means to comply with the BASIC readiness
immunization standards.
- Each
immunization must be listed on your OFRD Officer Summary Page.
Addtionally, list ALL doses received for multi-part series. For example, Hepatitis B, list each of the
following on a separate line: Hep B dose #1, Hep B dose #2, Hep B dose
#3, and a Hep B surface antibody titer.
- MMR
(Measles, Mumps, Rubella)
1) Documented positive titers for (all three) measles, mumps, and
rubella OR
2) Two doses of MMR vaccine OR
3) fulfilling any one of the three requirements for each of the
following three illnesses:
2) 2 adult doses at least 4 weeks apart
OR
3) valid Measles waiver
- Chickenpox
(Varicella )
1) documented positive titer, OR
2) 2-dose vaccination at least 4 weeks apart as adolescent or adult (13
years) OR
3) valid Varicella waiver
1) medical record documenting vaccination within the last 10 years
OR
2) valid Tetanus/Diptheria waiver
1) medical record documenting vaccination between 1
October and 31 December of each calendar year OR
2) valid Influenza waiver
- PPD
(test for Tuberculosis)
Two negative tuberculin skin test (TST) results no greater than 12 months
apart
OR
A single negative interferon-gamma release
assay (IGRA) test (e.g., QuantiFERONŽ-TB Gold test) result is sufficient
evidence of the absence of infection with Mycobacterium tuberculosis (TB)
and no additional annual TB screening is required.
OR
In the absence of two negative TSTs within 12 months, the officer must
continue annual TST screening until this requirement is met or obtain a
single negative IGRA result.
Officers with a history of a positive TST or IGRA result and who have
submitted supporting documentation to the Office of Commissioned Corps
Support Services (OCCSS), Medical Affairs Branch (MAB), indicating absence
of active TB disease (i.e., medical evaluation including a chest radiograph
or positive IGRA result) comply with this TB screening requirement.
OR
Valid PPD waiver
1) medical record documenting two shot series of Hepatitis A, generally 6
to 12 months apart. OR
2) medical record documenting positive titer OR
3) valid Hepatitis A waiver
1) Medical record documenting history of the three shot series of Hepatitis
B OR
2) medical record documenting positive titer OR
3) valid Hepatitis B waiver
